Walk Away from High Blood Pressure
Surely everyone knows by now that regular exercise is vital in helping to prevent or control high blood pressure (hypertension). How much and how often, though, are questions that have been the subject of speculation and more than a few studies. Now comes one from the department of kinesiology at Indiana University and Indiana University Medical Center, Bloomington, with results that surprised even the study authors.
Researchers wanted to find out which is better for reducing prehypertension -- continuous physical activity, in this case walking, or walking that totaled the same amount of time but was performed in short spurts.
Prehypertension is blood pressure that runs from 120 to 139 mmHg (systolic) over 80 to 89 mmHg (diastolic), which are under the official level of hypertension. They discovered that 40 minutes of walking, whether in one continuous outing or in frequent spurts for shorter periods of time, reduced blood pressure by a similar amount compared with control subjects (a 5.4 to 5.6 drop in systolic pressure, and a 3.1 to 3.4 drop in diastolic pressure). However, when the group took shorter, more frequent walks (four times a day, in this study) the effect lingered for 11 hours, versus only seven hours when the group performed continuous walking.
I called the study's co-author, Janet P. Wallace, PhD, who assured me that both exercise groups did identical "work" (intensity) -- whether it was continuous or broken up throughout the day. Participants walked at 50% of their capacity (or VO2 max, a measure of aerobic fitness), which is a moderate pace. "Brisk" walking is working at 70% of capacity, she says, but in treating blood pressure with exercise, the ideal is moderate intensity and longer duration -- 45 minutes is optimal and one hour is max, according to Dr. Wallace, since after an hour there is no further measured short-term benefit. (Anything over 60 minutes is affected by the law of diminishing returns... you do not get that much more out of working longer, she said.) I wondered if part of the reason for the longer effect was the chance to relax several times a day, but Dr. Wallace says it more likely results from spreading out the physical stimulus over a longer period.
Her advice, based on the study, is to do whatever fits best into your life -- continuous or several briefer periods of activity in the day. And there's nothing magical about walking (other than how easily it can be done)... any kind of moderate intensity physical activity helps keep blood pressure down, she says. In a previous study her team discovered that even gardening and housework were helpful. The key is finding and doing activities that are easy for you to accomplish, and that you will do every day, be it 45 minutes on the treadmill at the gym or getting out of the house or the office every few hours for short walks outside.

Digestion Connection --
The Recipe for Optimal Cholesterol
We spend an inordinate amount of time talking about how to raise good cholesterol and lower bad cholesterol, but that discussion may well be missing the point. The body actually needs both kinds, and it's the balance between the two that is most important. I recently discussed this -- how the media, along with all those "cholesterol lowering" drug manufacturers, are delivering the wrong message -- with Daily Health News consulting medical editor Andrew L. Rubman, ND. So, today, Dr. Rubman explains the balance theory of cholesterol management.
"Cholesterol is necessary for life," says Dr. Rubman, "ergo it can't really be 'bad.'" It only causes trouble when it gets out of proportion, with too much LDL and too little HDL relative to each other. When it comes to total cholesterol/HDL ratio, the American Heart Association recommends a ratio of less than 5:1, optimally, 3.5:1. Dr. Rubman says the ratio is more useful than a fixed target number as people are prone to striving for." So, the real focus with cholesterol is -- or ought to be -- how to manage it better." In order to accomplish that, we need to understand what cholesterol is and what it does.
PROTEIN BUILDING BLOCKS
What we commonly call "cholesterol" -- both the 'bad' (LDL) and the 'good' (HDL) -- is actually a little container of protein and fat (called "lipo-proteins" -- lipo means fat) synthesized together by the liver. These help your body produce cell membranes, create estrogen, testosterone and other vital hormones. (See Daily Health News, June 12, 2003, for more information about the job of cholesterol in our bodies.)
Properly nourished, the body naturally balances HDL and LDL. However, our modern diet doesn't accomplish this so easily. Since the body works with whatever elements it has on hand to produce HDL and LDL, a first step to achieving balance is to eat and digest the right forms of protein.
What kind of protein should you eat? "High-quality, minimally processed food," said Dr. Rubman, suggesting you strive to include one "good high-value protein" at every meal. Dr. Rubman's favorite sources of protein include lean organic chicken or beef, and wild, deep-water fish, as well as beans and other legumes.
ABSORBING THE PROTEIN
Equally important (and less often understood) is that we need to make sure the protein is adequately digested and absorbed. "There's a huge digestive link to the whole issue of cholesterol," Dr. Rubman told me. "Protein can't be used if it isn't properly digested. It can't be digested properly if you don't break it up with your teeth -- after all, there's no grinder in your stomach." So it turns out that proper chewing can actually influence how much cholesterol your body will absorb and then synthesize. Depending on what's on your fork, aim for 30 or more chews per bite to insure optimal digestion.
Similarly, having adequate amounts of stomach acid will help ensure complete digestion of your proteins. This means that older people and regular users of acid-suppressing medications (both OTC and prescription) may be inhibiting their body's ability to fully digest protein.
In order to manufacture cholesterol optimally, the liver also needs B vitamins, Dr. Rubman said. For his patients, he prescribes a good, high-potency B complex vitamin to be taken at least twice daily, and often a separate vitamin B-12. Second, he advises regularly eating dark green, cruciferous foods (optimally one serving daily) like Brussels sprouts, broccoli and kale.
Garlic can also be helpful in balancing cholesterol. Even in the mainstream medical community, garlic is understood to be an agent for lowering LDL cholesterol, while some research has shown it to actually raise HDL as well. Garlic supplements are usually unnecessary, says Dr. Rubman. "For most people, cooking regularly with garlic is adequate and helpful."
Finally Dr. Rubman recommends fiber: "Many types of soluble fiber can bind cholesterol and carry it out of the body with the bowel movement," he explains. This is how the body eliminates excess cholesterol. As a supplement he often prescribes glucomannan or Konjac fiber, taken a half-hour before the largest meal of the day with a large glass of water.
One last recommendation from Dr. Rubman on achieving cholesterol balance -- exercise. It increases the metabolic use of lipids, enabling your liver to make more high-density lipoproteins (HDL).
THE RECIPE FOR BALANCED CHOLESTEROL
According to Dr. Rubman, if you're controlling your diet correctly by increasing your intake of good protein, perhaps spiced with garlic, and attending to your digestion by making sure you chew every morsel and are maintaining adequate stomach acid, you're well on the way to naturally increasing HDL cholesterol -- and creating the proper balance of HDL and LDL. Avoid highly processed carbohydrates, which can interfere with these benefits in numerous ways, both directly and indirectly. Add exercise, B vitamins, and some fiber, along with medical oversight, and that, says Dr. Rubman, "is a recipe for cholesterol success."

Fried foods creategastrointestinal inflammation and speed the aging process.Between-Meal DefensesIf you still suffer from a sour stomach between meals, put somethingin it that will quiet it without triggering more acid production. Secret Superfoods: Who Knew?
If I had a dollar for every time the conventional wisdom on nutrition has shifted gears, I'd be a rich woman. Remember butter? And how about eggs? First they're evil, now they're our best friends. I've watched the experts flip recommendations on so many foods I've lost count. Some of these foods suffered from bad reputations for years, but are now back on our buddy lists... others were unsung heroes, always good for us but never getting proper credit. I think it is a good idea to periodically revisit some of our cherished beliefs about foods and health, so I spoke with Jonny Bowden, whose recent book, The 150 Healthiest Foods on Earth (Fair Winds Press) has some surprising entries -- and some surprising omissions.
FAT CAN BE YOUR FRIEND
"Many foods suffered from bad reputations in the past because of the deeply held belief that all saturated fat is bad for you," Bowden said. "Saturated fat is the general name for a collection of fatty acids -- and some of these fatty acids are really good for you." For example, he says, "two of the healthiest foods on earth are whole eggs and coconut. Yet to this day people eat 'Egg Beaters' and shun coconut oil because of the saturated fat."
EGGS - GREAT PROTEIN SOURCE
Bowden believes whole eggs are among the finest sources of protein on the planet. "On three of the four methods used by scientists for rating protein quality, eggs score better than milk, beef, whey and soy," he told me. (All four -- milk, beef, whey and soy -- are tied on the other commonly used rating system.) "Whole eggs contain all nine essential amino acids, plus they're loaded with vitamins and nutrients that are excellent for your eyes, brain and heart." Bowden explained that the fear of saturated fat causes people to shun one of the healthiest parts of the egg -- the yolk. "The yolk contains lutein and zeaxanthin, two members of the carotenoid family that are emerging as superstars of eye nutrition. And the yolk is also one of the best sources of choline."
Choline, says Bowden, is a building block for important compounds in the body that help support the heart, brain and liver. "Choline creates betaine, which helps lower homocysteine, a risk factor for heart disease. It's part of a compound called phosphatidylcholine that helps prevent the accumulation of fat and cholesterol in the liver. It's also needed to make acetyl-choline, which is critical for memory and thought." Incidentally, he didn't perceive the amount of cholesterol in whole eggs as a problem. "Virtually every study has shown absolutely no link between eating eggs and heart disease," he told me.
NUTS FOR COCONUTS
Another surprising powerhouse is coconut, which Bowden believes is headed to the top of the food charts among knowledgeable nutrition gurus. "In my view, coconut and coconut oil are superfoods," he told me. A half-cup of shredded coconut has almost 4 grams of fiber, 142 mg of potassium and almost no sugar. As for the saturated fat, he explained that in coconut, 50% of it comes from a fatty acid called lauric acid, which is anti-viral and anti-microbial, and enhances the immune system. As for those saturated tropical fats that give coconuts a bad reputation... the saturated fat in coconut oil comes mostly from a family called MCTs or medium-chain triglycerides, making it particularly easy to metabolize. "The body likes to use it as a source of energy, rather than as a source of padding," said Bowden. "Long-term studies of people from the Pacific Islands who eat coconut and coconut oil regularly show that they have extremely low levels of heart disease."
BUTTER BARGAIN
Bowden even has kind words to say about butter. "It's a rich source of vitamin A, needed both for maintaining good vision and the optimal functioning of the immune system. Butter also contains other fat-soluble vitamins, like vitamin K and vitamin D. And when it is made from the milk of healthy, grass-fed cows, butter contains CLA (conjugated linolenic acid), a kind of fat that has proven anti-cancer properties."
HEALTHY BREWS
Long thought to be a health faux pas, Bowden told me that coffee is actually a major source of antioxidants in the American diet. "Caffeine is good for the brain," he explained, "and new studies show that it may be protective against diabetes and Parkinson's too. As long as you're not sensitive to the caffeine and you don't overdo it, coffee is fine for you." He also reminded me that black tea is nearly as good for you as its more popular cousin, green tea, because it, too, is loaded with flavonoids and catechins and antioxidants. But be aware that adding milk to your tea disables the antioxidants, so best to take it straight, with lemon or with your favorite sweetener.
NATURAL DISEASE FIGHTERS
Then there are the foods that are the unrecognized multitaskers in the health tool chest -- like pumpkin. It has more potassium than a medium banana, is loaded with vitamin A, and has two carotenoids that are beneficial for the eyes -- lutein and zeaxanthin. "Plus, pumpkin has less than 50 calories per cup, and you can season it with all kinds of great spices like cinnamon and nutmeg," adds Bowden. Another favorite is guava. "Guava has a whopping 9 grams of fiber per cup. It's another potassium heavyweight, and it contains the cancer-fighting compound called lycopene," he said.
Another unappreciated fruit -- cherries. These are loaded with quercetin and ellegic acid, two cancer-fighters. In addition, cherries have lots of natural anti-inflammatories, which is why they've traditionally been used to fight the pain of gout. While cherries are generally a summer treat, frozen cherries contain the same nutritional value as fresh, ripe fruit.

"Virtually every study has shown absolutely no link between eating eggs and heart disease," he told me.
NUTS FOR COCONUTS
Another surprising powerhouse is coconut, which Bowden believes is headed to the top of the food charts among knowledgeable nutrition gurus. "In my view, coconut and coconut oil are superfoods," he told me. A half-cup of shredded coconut has almost 4 grams of fiber, 142 mg of potassium and almost no sugar. As for the saturated fat, he explained that in coconut, 50% of it comes from a fatty acid called lauric acid, which is anti-viral and anti-microbial, and enhances the immune system. As for those saturated tropical fats that give coconuts a bad reputation... the saturated fat in coconut oil comes mostly from a family called MCTs or medium-chain triglycerides, making it particularly easy to metabolize. "The body likes to use it as a source of energy, rather than as a source of padding," said Bowden. "Long-term studies of people from the Pacific Islands who eat coconut and coconut oil regularly show that they have extremely low levels of heart disease."
BUTTER BARGAIN
Bowden even has kind words to say about butter. "It's a rich source of vitamin A, needed both for maintaining good vision and the optimal functioning of the immune system. Butter also contains other fat-soluble vitamins, like vitamin K and vitamin D. And when it is made from the milk of healthy, grass-fed cows, butter contains CLA (conjugated linolenic acid), a kind of fat that has proven anti-cancer properties."
HEALTHY BREWS
Long thought to be a health faux pas, Bowden told me that coffee is actually a major source of antioxidants in the American diet. "Caffeine is good for the brain," he explained, "and new studies show that it may be protective against diabetes and Parkinson's too. As long as you're not sensitive to the caffeine and you don't overdo it, coffee is fine for you." He also reminded me that black tea is nearly as good for you as its more popular cousin, green tea, because it, too, is loaded with flavonoids and catechins and antioxidants. But be aware that adding milk to your tea disables the antioxidants, so best to take it straight, with lemon or with your favorite sweetener.
NATURAL DISEASE FIGHTERS
Then there are the foods that are the unrecognized multitaskers in the health tool chest -- like pumpkin. It has more potassium than a medium banana, is loaded with vitamin A, and has two carotenoids that are beneficial for the eyes -- lutein and zeaxanthin. "Plus, pumpkin has less than 50 calories per cup, and you can season it with all kinds of great spices like cinnamon and nutmeg," adds Bowden. Another favorite is guava. "Guava has a whopping 9 grams of fiber per cup. It's another potassium heavyweight, and it contains the cancer-fighting compound called lycopene," he said.
Another unappreciated fruit -- cherries. These are loaded with quercetin and ellegic acid, two cancer-fighters. In addition, cherries have lots of natural anti-inflammatories, which is why they've traditionally been used to fight the pain of gout. While cherries are generally a summer treat, frozen cherries contain the same nutritional value as fresh, ripe fruit.
